ISOLATION AND IDENTIFICATION OF MICROORGANISMS ASSOCIATED WITH MOBILE PHONES
Abstract
I, the undersigned medical practitioner, hereby certify that the above-named Prospective Corps Member (PCM) presented at our facility for medical evaluation in respect of his fitness to fully participate in NYSC Orientation Camp activities.
The PCM reports a fracture of the left/right leg sustained following a road traffic accident approximately four (4) months ago. Clinical assessment and review of the patient’s medical history indicate that although initial fracture management was carried out, he is still experiencing persistent pain, limited weight-bearing ability, and reduced mobility.
Examination Findings:
Healing fracture site with residual tenderness
Reduced range of motion at the affected limb
Difficulty performing prolonged standing, marching, drills, and other strenuous physical activities required in the NYSC orientation camp
Ongoing physiotherapy and follow-up treatments still required for full recovery
Based on the above, the PCM is not currently fit to participate in compulsory physical activities within the NYSC orientation camp. However, he is considered stable enough to report for registration only, after which he will require immediate exeat from camp to continue medical treatment and avoid worsening of his condition.
Recommendation:
It is hereby recommended that the NYSC officials kindly grant the PCM an exeat after registration and excuse him from all camp-related physical activities until full recovery is achieved.
Thank you for your understanding and cooperation.
